NATIONAL HEALTHCARE DECISIONS DAY and HAWAII HEALTHCARE DECISIONS WEEK:
APRIL 13-19, 2008!
On April 16th, join Americans across the country to talk to others about your future healthcare decisions and to complete your advance directive. The 1st National Healthcare Decisions Day Initiative is a collaborative effort of national, state and communitiy organizations committed to ensuring that all adults with decision-making capacity in the United States have the information and opportunity to communicate and document their healthcare decisions. Now is the time to complete your own advance directive form, and urge loved ones to do the same.
