Five Wishes® Advance Directive
Five Wishes® is an easy-to-use advance directive (AD), a legal document in which you name the person you want to make healthcare decisions for you (in the event you no longer can) and state your wishes for healthcare treatment. You do not need a lawyer or a notary to complete it. Kokua Mau now offers Five Wishes® documents with lifetime electronic storage so that healthcare providers can access your AD anywhere.
